Turn-in Day: Monday, August 27, 2018
Please complete a Cobb County Schools Payment Envelope with the amount labeled on the front. On the “Other” line, please label as a music department donation. If your child is in chorus only, write “chorus donation”. If they are in two year-long classes, write both names on the envelope. Thank you for your contribution!
How to turn in your fundraiser order:
1. Fill out the Complete and Final Order Form. It is requested that adults complete this form so that it is completely legible. Parents must sign the first blank to verify that your order is correct. Please do not sign the second blank yet. This line will be signed when you pick up the items during Conference Week in October.
2. Turn in your Complete and Final Order Form with ONE form of payment (Cash OR one check made payable to Lovinggood Middle School). If you did not choose to submit funds with one form of payment, all checks must be made payable to Lovinggood. Cash is also acceptable. All money must be submitted in the Cobb County Schools payment envelope (not the large envelope from the company):

3. If you so choose, you may submit your brochure. It will be stored in a file cabinet until October’s pick-up date. It is suggested that you either make a copy of, or take a picture of ,your brochure order form for safe keeping. Brochures are not required to be turned in because your order is filled using the information on your Complete and Final Order Form. Please make sure that this form is correct before you sign and submit on Monday!
