INTENT FORM FOR 2020-2021

THE FOLLOWING INFORMATION WAS SHARED THROUGH TEASLEY’S BLACKBOARD SYSTEM YESTERDAY:

K-4 
As Teasley prepares for the upcoming school year, we must have accurate information on file for your student and family. We need your help ensuring that the information on file is correct, and to determine how many Teasley students will be returning for the 2020-2021 school year. Please click the link below and complete the form for EACH of your students. If your student is looping with their current teacher to the next grade level, we still need you to complete this form. This form must be completed for each student individually.
Also, for your child to be placed on a class roster for the 2020-2021 school year at Teasley Elementary, you must provide an updated lease, if your lease has expired or will expire before August 3, 2020. You must provide an updated lease and current utility bill prior to the first day of school. If any of these required documents are not received, your child WILL NOT be on a roster for the 2020-2021 school year. If you have moved outside of the Teasley Elementary attendance zone, and would like to remain, you must complete the Continuation of Enrollment form online at cobbk12.org. There will be a separate email with a separate form to complete for all 5th grade students (SEE BELOW).
Please have these forms completed and submitted no later than May 11th. Please email any lease and utility information to joy.warren@cobbk12.org. Thank you for your continued support during these digital learning days!

 

5th ONLY

It’s hard to believe that the school year is coming to an end and our fifth-grade students will be starting middle school in the fall. In order for us to help make your child’s transition to middle school as smooth as possible, please complete the attached form and submit no later than May 11th.

Please note that access to student records is still extremely limited at this time and we are unable to process any records requests until we are permitted back in the building for business as usual.
